At Future Family, Inc., we are committed to supporting you during your fertility journey. Our mission is to provide women and couples with the care, support, and expertise they need and deserve. We seek to help answer your questions, help you advocate for your own care, and to manage the complex journey of fertility treatment.


Further, we value your privacy. This privacy policy (“Policy”) describes the information we collect when you access or use our website https://www.futurefamily.com/ and related services (“Services”) and how we use, disclose, and protect this information. Your access and use of our Services indicate your consent to the terms of this Policy.


Information We Collect


Information You Provide


In order for us to provide you with our Services, we may ask you to provide us with or collect from third party sources, certain information, including:


  1. your name, location, and your contact information (such as your e-mail address, phone number, billing and physical addresses, and your account password);
  2. identification information (social security number)
  3. demographic data (such as your gender, your date of birth, and your zip code);
  4. financial and payment information (such as your income and asset information, credit or debit card number, bank account routing number, and billing and shipping address); and
  5. health history and details about your fertility journey.

We may, as needed, share this information with any healthcare providers (e.g. clinics, pharmacy partners, etc.) that you list on your loan application or loan contract with us. We may also share information about your loan such as credit decision (approval), loan amount and disbursement details.


If you communicate with us by, for example, e-mail, telephone, online form, or letter, any information provided in such communication may be collected. Further, you may provide us with your Google account information to receive Google calendar updates and notifications.


Information We Collect When You Use Our Services


Location Information

Some of the features of our Services may enable us to access your location in order to tailor your experience with our Services based on your location (“Location-based Services”). In order to use certain Location-based Services, you may be asked to enable certain features of your mobile phone, which enable us to identify your location through a variety of means, including GPS location, IP address, or cellular, Wi-Fi, or Bluetooth signals, as available. If you choose to disable any Location-based Services on your device, you may not be able to utilize certain features of the Service.


Information from Local Storage and Similar Technologies

We use local storage and may use other tracking technologies on our Services. Local storage is a file created by a website on your device used to retain data on your device across visits to the same website. Local storage helps us track and target the interests of our users to enhance their experience with our Services. Some of our business partners may use cookies, local storage or similar technologies on our Services. However, we have no access to or control over these technologies. For information about how to opt-out of or manage certain use of local storage and other technologies, please see the Your Choices section below.


Information from Clinics

In providing our Services to you, we help manage a shared calendar of the appointments you scheduled with the clinic. In doing so, we’ll obtain information about the dates, times, and nature of your appointments. If you authorize the clinics to do so, they will share with us your treatment schedule, which will include information about your appointments and medications. To the extent you are receiving services from a clinic or other entity subject to the Health Insurance Privacy and Accountability Act of 1996, as amended, and its implementing regulations (collectively, “HIPAA Covered Provider”) you must execute an authorization with the HIPAA Covered Provider on a form they provide before requesting that it provide any information to Future Family.


How We Use Information


We may use the information we collect in the following ways:


Provide & Improve Services

When you register for our Services, we use the information you provide to create and maintain your account. The information we collect also helps us improve, enhance, personalize, and promote our website, products, and services. You understand and agree that as part of the registration process that we will use the information you provide to verify your eligibility to use our Services.


Communications

We will communicate with you if you’ve provided us the means to do so. For example, if you’ve given us your email address, we may send you promotional email offers or email you about your use of the Services. If you do not want to receive promotional emails from us, please indicate your preference by opting-out via the links provided in the emails. Note, that if you opt-out of receiving our promotional emails, you may still receive transactional emails from us.


Surveys & Contests

From time-to-time we may offer surveys or contests. Participation in these surveys or contests is completely voluntary and you may choose whether or not to participate and provide the relevant information. Information requested in relation to these surveys or contests may include your contact information, demographic information, and other information you provide. Contact information will be used, for example, to notify the winners and award prizes. Survey information will be used, for example, to monitor and improve our Services and your satisfaction or to see if there are any useful trends in fertility that can help the fertility industry and journalists understand more about what people are thinking about and doing around fertility issues. All participants in surveys will be noted as anonymous.


How We Disclose Information


We may disclose the information we collect, including in the following ways:


Vendors & Service Providers

We employ other companies and people to perform tasks on our behalf and need to share your information with them to provide our Services to you; for example, we use a payment processing company to receive and process your credit card transactions for us. In addition, we may use third party analytics vendors to evaluate and provide us with information about your use of our Services. These analytics service providers may set and access their own cookies, pixel tags and similar technologies on our Services and they may otherwise collect information about you over time and across different online services.


Aggregate Data

We may de-identify your information and process, use, and share it in an anonymous and aggregated form for other purposes than described above.


Business Transfers

We may choose to buy or sell assets, and may share and/or transfer your information in connection with the evaluation of and entry into such transactions. Also, if our company (or our assets) are acquired, or if we go out of business, enter bankruptcy, or go through some other change of control, your information could be one of the assets transferred to or acquired by a third party.

California Online Privacy Protection Act Notices


We do not respond to Do Not Track (DNT) signals because we have not adopted a DNT standard. When you use our web site, third parties have the ability over time and across different Web sites to collect information about you and your online activities. Except as may be described herein, we do not authorize the collection of Personal Identifiable Information (PII) for third party use through advertising technologies.

Under California Civil Code Sections 1798.83-1798.84, California residents are entitled to contact us to prevent disclosure of certain information to third parties for such third parties’ direct marketing purposes; in order to submit such a request, please contact us at support@futurefamily.com. California residents are entitled to ask us for a notice identifying the categories information which we share with our affiliates and/or third parties for marketing purposes, and providing contact information for such affiliates and/or third parties. Transborder Transfer


Our Services are mainly geared toward U.S. residents. If you live outside of the U.S., your information may be subject to privacy laws that are different from those in your country of residence. Information collected within the European Economic Area (“EEA”) and Switzerland may, for example, be transferred to and processed by third parties identified above, located in a country outside of the EEA and Switzerland, where you may have fewer legal rights in relation to your information.

We may display advertisements or complimentary offers from third parties and other content that links to third party websites. We cannot control or be held responsible for third parties’ privacy practices and content. Please read their privacy policies to find out how they collect and process your personal information.


Security


We are committed to protecting your information. We seek to use reasonable organizational, technical, and administrative measures to protect information within our organization. Unfortunately, no data transmission or storage system can be guaranteed to be 100% secure. Your password protects your user account, so you should use a unique and strong password, limit access to your computer and browser, and log out after having used our Services. Children


You must be 18 or older to use our Services. Our Services are not directed to children under the age of 18. We do not knowingly collect personal information from children under 18.

From time to time we may make changes to this Policy.
